Job Description & Responsibilities:
Evaluate patient needs and determine appropriate referral options based on clinical guidelines and patient preferences. Coordinate and schedule referrals with healthcare providers, specialists, and community resources to ensure timely access to services. Maintain detailed and accurate records of all referral activities, communications, and follow-ups in compliance with healthcare regulations. Communicate effectively with patients, families, healthcare teams, and external agencies to provide updates and resolve any referral-related issues. Monitor referral outcomes and follow up to ensure patients have successfully accessed recommended services and support. Educate patients about available healthcare and social services to empower informed decision-making. Collaborate with insurance companies to verify coverage and assist patients with authorization processes when necessary.
Qualifications:
Minimum Qualifications: High school diploma or equivalent required; Associate’s degree or higher in healthcare, social work, or related field preferred. Experience working in a healthcare or social services environment, preferably in a referral coordination or case management role. Strong knowledge of healthcare systems, insurance processes, and community resources. Excellent communication and interpersonal skills to interact effectively with diverse patient populations and healthcare professionals. Proficiency in using electronic health records (EHR) and referral management software. Preferred Qualifications: Certification in case management or patient navigation. Experience working with vulnerable populations, including elderly, disabled, or low-income patients. Familiarity with HIPAA regulations and patient privacy standards.
