Job Description & Responsibilities:

The Community Navigator is responsible for assisting and facilitating the application process for Medicaid, waivers, housing, food stamps, entitlements, and other government programs. The role is responsible for ensuring that individuals and families in need have access to vital resources and support services. The individual will collaborate closely with the care management team and follow the person-centered plan of care as an outline for the individual’s needs and the required interventions.

Qualifications

High School Diploma required. Bachelor's degree in Social Work, Human Services, Public Administration, or 4 years equivalent years of relevant work experience is required. Minimum of 2 years of experience in social services, case management, or a related field, with a focus on government program applications. In-depth knowledge of the local state Medicaid program and benefits, Medicare, waivers, housing assistance programs, food stamps (SNAP), and other local state entitlement programs. Bilingual skills preferred.
